Please help!! leg pain

My mother is 54 years old she has severe pain and numbness in her left leg which extends down to her foot she can't walk for more than 5 minutes and at times she also feels heaviness together with numbness at the back of her head she got her MRI of L4-5spine done on the recommendation of a neurophysician and her result is as under:

Circumferential disc bulge at L4-5, which in combination with ligamentum flavum hypertrophy is causing moderate central canal and severe bilateral recess narrowing.

Please can you help and tell me what exactly this means and will it require a surgery or only physiotherapy would be of help.

I will be highly grateful if u can help me out with this as soon as possible.

Hi Sarah and Welcome

Looks like a doozy to me. Keep in mind that is my opinion and it's as scientific as it sounds. (I far from being a doctor.) Spine Health members can only give advise based on personal experience, but I suggest that your mom see a neurosurgeon and on orthopedic surgeon for a few opinions on what her options are. I had pain that sounds similar to your mom's pain and I had a fusion. There are others here who will tell you the same thing about the pain, but will have had other treatments. Has your mom tried any treatments such as physical therapy, injections, medications? Check out this link that can help you understand some of the information you may run into.

it sounds to me

This is just my opinion, but it sounds to me that your mom is dealing with sciatic pain. when there is a herniation at that level, mine was L5-S1, it can pinch the sciatic nerve that runs down the leg and into the foot. For me, this caused severe electric like pain from my hip to my foot along with some numbness and tingling. Whether or not she needs, or opts for surgery is up to her and the neuro. I eventually had the surgery because I was starting to lose feeling in my foot altogether. As you can see in my signature, I have since had a lot of problems. It is different for everyone and they may have her try some less invasive treatments first like I did. These would be things such as epidural steroid injections to try to reduce any swelling, physical therapy, or trigger point injections. I personally would highly suggest trying all of the things available before jumping right into surgery unless of course the neuro says that is the only option or something. You definitely want to listen to the neuro but at the same time, if you and your mom don't feel totally comfortable with what they say, don't hesitate to get a second opinion. Many people here have been through this and can offer some very good advice, the operative word here being advice. None of us can tell you or your mom exactly what is right for her but we can at least let you know what we have tried. Herniated disc (L5-S1) with sciatic pain, Laminectomy in 2006. Post-op chronic lower back pain which I did not have before the surgery and no one can figure out why. Started having chronic widespread pain shortly after first surgery as well as the back pain. Diagnosed with Fibromyalgia in 2007. Discovered DDD in 2008. Fusion at L5 in December 2008 with hardware. Current meds: 80 mg Oxycontin 3 x a day with an additional 20mg as well as the 80's 3 x a day since they don't make it in 100mg. 10mg percocet. Baclofen. Klonopin. Nortriptyline. Requip. Celexa.
Past meds: Neurontin and Lyrica,both made me sick. Savella which is the first drug made specifically for Fibro and not just as a secondary...went in to anaphylactic shock. Oral morphine, Avinza and Kadian, both made me sick even though the IV Morphine never did. Fentanyl patch made me hallucinate, I swear I could see through my eyelids, LOL! Oral Dilaudid made me a little queasy and tired, no pain relief. Lidoderm patches & trigger point injections, didn't help. Failed ESI's. Open to any suggestions. Lauren.

Your question can really only be answered by her doctor but unless there is a risk of permanent damage they will normally try all of the conservative steps first. That is great that you are being so supportive of your mom. If you need support for yourself do not hesitate coming here as we are pretty good at supporting one another. Good luck and let us know how your mom makes out.
