Medical History

Back injury at birth this has never been confirmed that this was the cause of my back problems and there is no documentation for me to research. I am only going by a journal my mother wrote while I was a baby and stories from my family. I started going to chiro at about 5 days old and continued on and off until I was about 11. Started going back to chiro about once a year when I turned 21. I seemed to have a "flare up" about once a year and was able to recover in 2-3 months of chiro treatment. During this whole time xrays, MRI and other tests were not ordered. I also tried orthopedic inserts due to a lack of arches in my feet and the chiro thought this could releif some of my pain. Went through a couple months of decompression therapy in 2006 which seemed to make the pain worse. Referred to a Orthopedic surgeon by my primary care doctor in 2007. MRI showed mild bulge of L5-S1 disc but he said it was not too bad. Referred to PT for 4 months with no releif then referred to Pain Mgmt doctor. No releif to several different injections referred back to orthopedic surgeon. Ortho did discogram which confirmed L5-S1 was bad and spinal stenosis. Ortho recommended artificial disc replacement and after appealing my insurance company, I finally gave in and had an L5-S1 anterior discectomy and fusion with interbody carbon fiber spacers, anterior plate and cancellous screws 7/1/08. 11/19/08 CT Scan shows L4-L5 disc is minimally degenerated with diffuse bulging, no disc hernation or spinal stenosis. Facet joints are very minimally degenerated with posterior marginal spur formation. L5-S1 shows mild bulge after fusion and discectomy, no disc herniation. Central Spinal canal is mildy narrowed on a congenital basis. Neural foramina are patent? Small posterior facet spurs are seen. Sacro-illiac joints are normal in appearance. 12/4/08 got SI Joint right side and trigger point injection, no releif. 2/1/09 EMG shows no evidence of nerve damage or impingement. 3/24/09 New patient visit with Neurosurgeon for 2nd opinion and 2 Dr Appts for SSDI.
